Austin Family Medicine Patient Forms

We are excited that you are interested in becoming a new patient at Austin Family Medicine Associates. We have included our new patient paperwork online, so you can fill these forms out from the comfort of your own home before getting to our office. If you have a PDF Reader (download Adobe Reader FREE), you can fill these forms out on your computer. After you have completed them, you can print them out. Please note that you will not be able to save the information that you have typed in the forms unless you have purchased Adobe Acrobat. If you do not want to type on the forms, you can print the forms out and handwrite in the blanks. We cannot receive these forms electronically because it contains protected health information which is regulated by HIPAA (a federal organization). At this time, we cannot accept “electronic signatures.” We ask that you sign each form with a pen.
